Teen Girl Fall Retreat Required Documents
All attendees of the Fall Teen Girl Retreat must have a completed Release Form.
All adult chaperones must have completed Live Scan fingerprinting under the AGNCN District ORI #. This needs to be completed at least two weeks prior to the retreat for clearance.
All adult chaperones must have on file a completed AGNCN Volunteer Form. This needs to be submitted prior to the retreat.
All adult chaperones must have completed the State Mandated Reporter Training and presented the certificate. If you have done this training already, you only need to present the certificate. This needs to be submitted prior to the retreat. Click the button to be linked to the website.
This form is to be used ONLY for necessary special diets due to MEDICAL restrictions. You will be notified prior to the retreat if we are unable to accommodate your request. Thank you!
